Some of the committee members have made comments about how other similar accidents may be used to inform future improvements. Comparing Deepwater Horizon and Costa Concordia may be a logical starting point , given the following similarities are: a low probability but high consequence accident; a captain who did not appear to understand the gravity of the situation as it unfolded nor magnitude of the risk being taken; encouragement from management to take risks that result in saving time and improving the bottom line; lack of effective safety oversight of the vessel’s master from a shore‐based or industry‐wide or regulatory body; failure to fully utilize available technologies that have the ability to monitor safety problems, predict unsafe paths; warn and identify risks; etc.
